Valid Insurance Certification for proof of one million combined single limit insurance
Photo copy of your valid CDL License
Photo copy of your valid DOT Medical card
Annual inspection report of your truck(s)
Signed completed lease
Copy of valid Workers Comp. Certification
Signed copy of W-9.
